Registering with the surgery

To register with the surgery:

When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.

You may still need to come into the practice to sign the forms and provide evidence of your identity and address (if you have them).

Once you have registered, you must book a new patient health check so we have all your information as soon as possible.

Named accountable GP

All our patients have an allocated named GP, which is your registered GP. If you wish to know who this is, please ask at reception.

Please note that you can still choose to see the GP of your choice.

Poplars is a Safe Surgery

Everyone in the UK has the right to free care from a GP. Poplars Medical Centre is proud to be a Safe Surgery for everyone in our practice area. If you don’t have ID or proof of address and you live in our practice area, you can still register with us. We won’t ask for immigration documents.
