Sick & Fit Notes

If you’ve been off sick for more than 7 days, you can request a sick (fit) note

To request a sick note please fill out the form by clicking the link below, please fill out all sections or your note may be delayed. Unless you specifically request otherwise your note will be sent to you as a text message. This service is only available for follow on fit notes where a prior fit note has already been issued by the GP practice, not first requests for fit notes or for new problems. First fit notes require a consultation with a clinician.

For first fit notes, you will need to book an appointment. 

Follow-on fit notes can be requested through the online form. Please do not request a new fit note until your current one has expired as we will not be able to issue one until this point.

If you have had a hospital procedure, you should request a fit note from the hospital team.

If you’ve been off sick for 7 days or less

If you’re off work sick for 7 days or less, your employer should not ask for medical evidence that you’ve been ill. Instead, they can ask you to confirm that you’ve been ill. You can do this by filling in a form yourself when you return to work. We call this self-certification.
